Our Mission

We work to close the health and social gaps that hold families back. CHAP finds people who face real barriers, connects them with the support they need, and tracks outcomes so we know lives are truly improving. We believe strong communities are built when Community Health Workers have the tools, training, and structure they need to guide families toward better health.

About Us

CHAP is the nationally certified Pathways Community HUB for Richland County and our surrounding region. At its core, the HUB model focuses on reducing risk. We look beyond symptoms and take a full view of a person’s life—housing, food access, transportation, safety, employment, education, and more—because all of it shapes health.

Our approach is simple: find, support, measure.We identify individuals who are at risk, complete a comprehensive assessment, and turn each identified barrier into a “Pathway”—a structured plan that follows a problem from start to finish. Each completed Pathway represents a real change in someone’s life, and every outcome helps us understand what’s working and where our community still needs to grow.

To make this happen, we partner with local agencies that employ Community Health Workers (CHWs). CHWs meet families where they are—often in their homes or by phone—and help them navigate complex systems that can feel overwhelming. They open doors to medical care, safe housing, education, employment support, and other essential services.

CHAP doesn’t replace the great work already happening in our community. We bring it together. Our HUB strengthens coordination, reduces duplication, and makes sure families don’t fall through the cracks. When barriers are removed, families thrive—and when families thrive, the whole community moves forward.
