Our Patient Assistance Fund helps patients in need access essential items like formula, diapers, toiletries, supplies, and transportation so they can focus on healing.

$100 Donation - Provides a Pack n' Play for ONE family

$200 Donation - Provides a week's worth of formula for FIVE infants

$500 Donation - Provides a month's worth of diapers for FIVE children

$750 Donation - Provides help with car repairs for THREE families

$1,000 Donation - Provides new shoes for 25 children

Third Street Family Health Services is a community health center based in the counties of Richland, Ashland, Crawford, and Marion, OH. Established in 1994, our community health center began as a part-time evening clinic with a staff of volunteer physicians, nurses, and receptionists. We quickly became an integral part of the health care delivery system for the area.  Over our history, we have continued to grow in response to community need with an emphasis on creating greater primary care access.  By the end of 2024, Third Street had over 100,000 visits and saw 24,000 patients in medical, dental, OB/GYN, and behavioral health visits.

Third Street empowers patients with help to address barriers such as mental illness, lack of transportation, and substance abuse. We are proud to provide behavioral health, community health worker, dental, medical, medication-assisted treatment, OB/GYN, pediatric, and podiatry services across multiple campuses in northcentral Ohio.
